Role Overview - Health & Wellbeing Practitioner HMP Brixton: In this fantastic role, you will be responsible for delivering client-centred treatment that employs harm reduction and abstinence-based methodologies, tailored to the individual needs of each client. Your day-to-day responsibilities will encompass caseload management, offering appropriate harm reduction guidance, conducting backssments, and creating care plans. A large part of this role involves collaboration with a variety of services including Transitional, Housing, Employment, Gym, Healthcare, Probation, and external Drugs Intervention Teams. Responsibilities – Health & Wellbeing Practitioner HMP Brixton: Create and oversee care plans addressing Substance Misuse and overall Health and Wellbeing needs for service users, aiding them in devising release plans that align with their individual.
