They employ a collaborative, interdisciplinary approach to evaluate and guide participant care, in accordance with their values, culture, and mission. Among other services, they offer medical care coordination, dental care, vision care, audiology services, pharmacy delivery, in-home primary care, physical therapy, and more.POSITION DESCRIPTION: The physician will work within a program of all-inclusive care for the elderly (PACE), which provides comprehensive medical and social services to eligible at-risk participants living in the community. Most participants are dually eligible for Medicare and Medicaid/Medi-Cal.Within the program, the physician performs in-person comprehensive assessments and coordinates with an interdisciplinary care team to develop individual care plans for participants.
