SUMMARY Mid-Ohio Food Collective (MOFC), Mid-Ohio Farmacy is a data driven initiative based on research that tell us food is medicine - what we eat is part of what helps us stay healthy. Eating more fresh fruits and vegetables, especially for people who have diabetes, high blood pressure, and struggle with obesity, can improve health and save money on health care costs. As such, Mid-Ohio Farmacy was created as a bridge between the Mid-Ohio Food Collective and healthcare providers with two goals in mind: help food insecure patients access foods needed for better health and improve health outcomes for individuals and families. We currently work with Federally Qualified Health Centers (FQHCs), Insurers, and health systems across Ohio to achieve these goals, enrolling over 30,000 patients to date. Patients who have joined Mid-Ohio Farmacy can get fresh fruits and vegetables one time each week at Mid-Ohio Farmacy food pantries for their entire household. Building on the success of increasing access to fresh produce for food insecure households, we are expanding our focus into health promotion and wellness, taking on new challenges to provide services and connections to our customers that can change behavior and improve health outcomes and at scale. Addressing other social determinants of health (SDOH) beyond food insecurity can also improve patient health outcomes. The next phase of our work seeks to holistically address food insecurity as a piece of that larger SDOH picture and improve health outcomes for our neighbors. The Mid-Ohio Farmacy Program Coordinator is r esponsible for assisting and coordinating the administrative aspects of the Mid-Ohio Farmacy program including enrollment and engagement of current Farmacy customers, reporting and ongoing communication and planning with healthcare partners, internal coordination of data from health partners to MOFC, and assisting the Farmacy team with ongoing research projects. The Mid-Ohio Farmacy Program Coordinator, in partnership with the Director of Nutrition and Research and the Senior Project Manager, will be involved in all aspects of the team's work - finding more creative ways to achieve health education and promotion at scale, and continue to grow the Farmacy program beyond healthcare referrals to end hunger and improve health outcomes. PRIMARY DUTIES : Track inventory of program materials and coordinate new material ordering when necessary. Prepare materials for new and current healthcare partners. Data entry and coordination of reporting to healthcare partners. Monitor and improve tracking system of data file transfers from partners. Conducting mailings and text message campaigns to Mid-Ohio Farmacy customers. Liaise with current Farmacy customers who have questions about program membership and benefits. Help enroll and track patients who are involved in current research study. Work with Senior Program Manager to ensure deadlines for invoicing are being met. Assist in web design/content for Farmacy site, and work with marketing team to coordinate social media campaigns. Other duties as assigned. EDUCATION & EXPERIENCE: Education and Certifications: Bachelor's Degree preferred or equivalent experience. Background in Public Health, Nutrition, Health Education or other related field is preferred. Candidate must have a valid Driver's License. Knowledge on program management/planning, data entry and analysis, and budgets is preferred. Experience: Two (2) years of e xperience working with stakeholders and customers is required - working with individuals in a healthcare setting is strongly preferred but not required. Experience with data entry and database management is highly desirable. Knowledge of issues around food insecurity and public health in Ohio is also highly desirable. Human subject research experience - both qualitative and quantitative is preferred. Skills/Competencies: Committed to the organizational mission of ending hunger. Strong project management and organizational skills are essential as are strong customer service skills, interpersonal skills and excellent written and verbal communication skills. Ability to work effectively and efficiently both independently and in a team environment. Proficiency in Microsoft Office software including Word, Excel, PowerPoint and Outlook. Must be able to manage multiple projects, meet deadlines and be flexible to changing priorities. Must be able to work cross-functionally, cross-culturally and build relationships that collaborate effectively with partner agencies, customers, and stakeholders. JOB CONDITIONS : Working Conditions : Normal office environment. Hybrid work schedule (3 days in the office, 2 days remote) is offered for this position. Occasional travel may be required.
