Early Help Lead Practitioner - Normanton Are you a passionate, dedicated Early Help Lead Practitioner looking for your next role 4Recruitment Services are recruiting an Early Help Lead Practitioner to join an Early Help Team based in Normanton. Umbrella Rate: £19.91 per hour Duration: 25/12/2023 – 3 months initially Hours: Monday to Friday, 08:30 - 17:00, 37 hrs/week – Office based Early Help Lead Practitioner role: Full JD Available Direct work with children and families Support the delivery of the Early Help Offer for families in order to reduce risk, increase resilience, and improve outcomes Case management of packages of support for families with multiple needs Undertake backssments, developing plans of support, and taking a lead co-ordinating role Support the delivery of effective interventions for change Work as part of a multi-disciplinary team Working with other agencies to improve identified outcomes Co-ordination of intensive multi-agency support packages where needed and leading multi-agency reviews of the support plans Respond to Early Help referrals received to determine the immediate needs of a child, young person or family Support information gathering to enable appropriate allocation Report on outcomes as part of the Children and Young People’s Performance Framework Engage with individuals, parents, children and young people presenting with challenging and risky behaviours of using or accepting services Act as lead officer for a thematic area determined by the team manager, acting as first point of contact Support other practitioners to ensure families receive good quality appropriate services Participate in supervision and appraisal Facilitate and attend meetings, reviews and conferences where necessary Essential Requirements UK Driving Licence, access to a vehicle, business insurance Recognised Level 5 or Degree Level qualification in a relevant subject Substantial experience in working with children, young people and families on an individual.
