The Service Coordinator is responsible for the service provision and management of a particular program under the direction of a Program Director, or an Administrator This position requires professional knowledge and skills sufficient to evaluate program functioning and to stimulate program development. It requires the ability to document services, prepare reports and manage the physical resources of the program. The position requires training and supervision of various levels of direct care staff. It requires being available for special events and in times of emergency. Minimum qualifications Be able to lift up to 50 lbs, be able to walk, bend, squat, turn in the knees, back, wrists and shoulders, and grasp with both hands (in order to properly perform the techniques taught in CPR/First Aid, Client Intervention Techniques, and Lifting and Carrying Techniques); Must be able to see and hear with or without correction. Be able to speak, read, and write English. Be able to pass a competency test. For some positions, employees must be able to pass a TB test. Knowledge, skills and abilities Experience w/patients Ability with lifting weight Good communication and written skills Essential functions Manage the day-to-day service provision of the program. Directs the implementation of policies and procedures in compliance with agency, funding, licensing, certification and accreditation. Establishes and maintains effective communication with all levels of staff, state personnel, advisory board, community groups, clients, and volunteers. Conducts group classes by managing the activities appropriate to clients' age and levels of functioning. Manages personnel activities in conjunction with the administrator. Provides input on preparation of program budget. Submits required reports and documents services. Document and monitor Therap documentation. Participates in long and short range planning for the program. Performs other duties as directed by the supervisor. Assist, prepare for and participate in emergency evacuations or shelter in place.
