Description:

Youable Emotional Health (formerly Headway) is seeking a Diversion Case Manager to work with first-time non-violent offenders to reduce their involvement in the juvenile justice system to improve the lives of children and their families (ages 10 to 17). Youth are identified and referred by the Hennepin County Attorney's Office to Headway. Headway offers community based assessment, counseling, and support services to youth and families. We engage in a holistic approach of assessment by looking at risk for further offenses, school engagement, family support, mental and chemical health concerns. Once assessment is complete youth are referred to supportive services within their community to address concerns identified as well as asked to complete community service and provide restitution if required. Youth are monitored and supported by the Diversion Case Manager through completion of the program. Location: Travel between Richfield and Brooklyn Center ESSENTIAL JOB FUNCTIONS Engage youth and families in the Diversion process through phone calls, face to face meetings, and connecting clients/families to community resources as stated in client's Diversion contract. Be available to clients via phone as needed. Completes assessments and initial screening with youth and families to determine needs and risks within 30 days of referral. Develop diversion contract with youth and families during intake process. Develop restitution contracts with youth and victims. Provide safety and crisis planning. Monitors progress on diversion contracts and restitution contracts; makes recommendations based on progress. Maintain relationships with community partners and schools. Collaborate with partners at the Hennepin County Attorney's Office. Attend Diversion meetings at the Hennepin County Juvenile Court. Monitor school attendance Completes all documentation and data entry required for the program in a timely manner. Administer and collect urine samples, as needed. Attends consultation, staff, and other meetings as directed by the supervisor or agency policy. Follow and enforce all agency policies and procedures. QUALIFICATIONS Required Bachelor's degree in one of the behavioral sciences from an accredited college or university Experience and training in working with families, work with youth involved in corrections a plus but not required Experience and training in identifying and assessing a wide range of children's issues and needs Knowledgeable about local community resources and how to utilize these resources to meet client needs Must have working vehicle and auto insurance Able to work evening hours Preferred, but not required Work with youth involved in corrections a plus but not required Fluent in Spanish Knowledge, Skills and Abilities Proven ability to establish and maintain positive rapport with a diverse variety of clients and professionals in school and community settings Ability to develop and maintain rapport with diverse and challenging youth Demonstrated excellent verbal and written communication skills Demonstrated ability to collaborate and build relationships Proficient computer skills in Word and Excel programs High degree of organization and ability to work independently and take initiative Ability to collaborate and utilize the skills of program team and other staff Ability to manage time efficiently in order to ensure paperwork is completed on time and completed correctly We are a passionate non-profit living out our mission by making a difference in the lives of thousands of families and individuals throughout the Twin Cites metro area each year.
