The Director of Social Services team member at Elderwood at Ticonderoga assists with ensuring the health and well-being of our residents by being responsible for the admission of residents and providing social services for designated residents. This a Full-Time position! In conjunction with this position, responsibilities will include oversight for the Adult Home (Elderwoood Village at Ticonderoga). This is a Part-time position in combination with the Director of Social Services. Responsibilities Director of Social Services/Adult Home Administrator Responsibilities: Assumes responsibility for admission process for new residents as described in Section B of the Social Services Department Manual. Serves as the Unit Social Worker for designated residents as described in Section B of the Social Services Department Manual, or delegates this responsibility according to current staffing pattern. Serves as the Discharge Planner of the facility as described in Section B of the Social Services Manual; completes SCREEN upon discharge of resident. Coordinates PASARR Level II Annual Review of residents. Implements and oversees the policies and procedures of the Social Services Department in the areas of admission, social work, transfer and discharge services. Participates in development of a comprehensive, inter-disciplinary health care and discharge plan for residents; attends Resident Care Planning and Resident/ Family Care Plan Review meetings and informs other disciplines of residents or family members who plan to attend. Provides support and assistance to the Unit Social Worker in work with residents and families. Supervises and evaluates the performances of the social services staff. Works with the Social Work Consultant in matters of social services intervention or consultation. Ensures all necessary documentation required for the Social Services Department. Interprets to the professional staff of the facility the purpose and objectives of the Social Services Department. Explains the Bill of Rights for Residents and Resident Abuse Reporting at General Orientation meetings, as scheduled. Coordinates facility cooperation with the Ombudsman. Direct and oversee facility operational activities of the Adult Home Possess understanding of organizational policies and procedures including DOH regulations Promote and guide initiatives to improve facility operations to meet goals and objectives Be able to multi task and manage multiple objectives simultaneously Possess ability to lead others and inspire them to achieve desired results Provide daily management and training of all staff, and the management of services through consultants and other contract arrangements. The provision of quality health care and daily living services for residents in conformance with state and federal laws. The provision of care and treatment of residents that promotes respect for the individual and the protection of basic rights. Maintain a safe, sanitary and pleasant environment for residents, visitors, and volunteers and good working conditions for employees. Ensuring resident census remains at capacity, controlling of costs for financial stability, and the safe-keeping of resident account and cash monies. The provision of payroll, billing and management information to the central office, Elderwood Administrative Services LLC. Encourages staff involvement, customer focus, leadership initiatives and sets standards for all staff. Qualifications Director of Social Services/Adult Home Administrator Qualifications: Baccalaureate Degree in social work or a related field from an accredited institution. Casework experience required. Casework with the elderly or work in a supervisory capacity requiring initiative and responsiveness to the needs of others preferred. Minimum of three years' experience in management, preferably a nursing home operation required. Qualities of leadership, dependability, integrity, and organizational ability necessary. Ability to communicate well verbally and in writing required. An interest in the field of health care, particularly the needs of the geriatric population necessary. Ability to meet the public easily, handle problems diplomatically, and cooperate with other staff members necessary.
