Key Responsibilities: Conducting comprehensive backssments of patients' health needs and developing personalised care plans. Delivering a range of nursing interventions, including wound care, medication management, and symptom management. Providing emotional support and practical advice to patients and their families. Collaborating closely with multidisciplinary teams to ensure coordinated care delivery. Maintaining accurate and up-to-date patient records and documentation. Contributing to the evaluation and improvement of service delivery through feedback and reflection. Requirements: Registered Nurse RN qualification with active registration with the NMCPrevious experience working as a Community Nurse Must be able to drive and have access to a vehicle.
