Community Health Worker HCHC is a non-profit, federally qualified health center with a mission to create access to high-quality integrated health care and to promote well-being for individuals, families, and our communities. We provide medical, dental, behavioral health, eye care services, and community programs to everyone, regardless of ability to pay. Compensation: competitive with a $1500 sign-on bonus available! Location: Worthington Health Center, Huntington Health Center, Community Center RESPONSIBILITIES Community Health Workers are public health workers who apply their unique understanding of the experience, language, and/or culture of the populations they serve in order to carry out one or more of the following roles. Providing culturally appropriate health education, information, and outreach in community-based settings, such as homes, schools, clinics, shelters, local businesses, and community centers. Bridging/culturally mediating between individuals, communities, and health and human services, including actively building individual and community capacity. Assuring that people access the services they need. Providing direct services, such as informal counseling, social support, care coordination, and health screenings; and Advocating for individual and community needs. QUALIFICATIONS CHW Core Competency Training strongly suggested, but will consider supporting candidates who display an aptitude for this work in obtaining certification qualifications. Culturally and linguistically qualified to work within population. Demonstrated commitment to impacting at risk patients Ability to work independently and as a team member across multicultural settings Detail-oriented with the ability to multitask Computer literate, experience with electronic medical record preferred Strong time management, organizational and planning skills Reliable transportation required and valid driver's license. Knowledge of area culture and resources required.
