The Case Manager will be responsible for developing and supporting programming for clients to help them achieve goals that will contribute to creating the life they want with support through adult programming. Our program helps individuals develop independent living skills, engage in activities they are interested in and navigate the community in which they live to make these opportunities sustainable.
