Job ID: 111265 Facility Group: Norton System Services Job Description Responsibilities Current Openings at the following hospitals: Norton Brownsboro Norton Women's and Children's Norton Hospital The Clinical Pharmacist Specialist role is to provide evidence based collaborative patient centered care to Norton Healthcare patients. The clinical pharmacist specialist optimizes medication therapy by championing and developing system wide strategies that lead to desired clinical outcomes while reducing/minimizing unintended adverse drug events and cost. Working collaboratively with physicians and other members of the healthcare team, this pharmacist will: Develop programs/guidelines that improve practice and patient outcomes; Provide efficient cost effective patient centered care; Provide verbal and written consultations concerning medication therapy on a daily basis; Participate in implementation and maintenance of guidelines and order sets; Participate in the educational experience of pharmacy residents and pharmacy students; Provide education to physicians, nurses and other members of the health care team; Provide presentations, publications and other informative activities related to specialty practice; Detect, monitor and report adverse drug events; Represent the pharmacy department on applicable committees and teams; Provide input and guidance for management of medication formulary; Provide operational support to the pharmacy, including dispensing and compounding. This position requires a high level of interaction with physicians, nurses, patients, other health care providers as well as pharmacy management in problem solving and project interaction. The role is responsible for: supporting and facilitating the QI process through ongoing development and monitoring of performance improvement indicators; growth and facilitation of Pharmacy work teams to meet quality, clinical standards of care; assisting the Director to provide effective and efficient use of all resources and for ensuring budget and value proposition targets are met. Work involves non-standardized tasks requiring professional judgment. Focus on reduction in errors which could lead to cost and quality concerns for the Pharmacy Department.
