A DAY IN THE LIFE OF A CASE MANAGER Provides case management services to clients referred through the criminal justice system inclusive of orientation, client education, crisis intervention as it arises and referral to appropriate outside agencies and resources upon release. Coordinate with the assigned counselor any possible candidates for the vivitrol program. Compiles monthly progress reports to referral source. Maintains electronic health record documentation on all clients that include reports, records, and consultation with other referral sources and social service professionals. Completes case management plans or individual treatment plans as required. Observe random drug screens and maintains results. Interacts and communicates pertinent authorized information to the local criminal justice system, treatment providers, and all those parties that work with TASC as a service to the client. Completes no less than twenty (20) hours of mandatory training per year. Participates in organized client staffing as needed. Maintains productivity in accordance to program expectations. Follows all Agency's policies, procedures and regulations; supports the overall mission and philosophy of the Agency. Performs other duties as assigned. QUALIFICATIONS FOR A CASE MANAGER Bachelor's Degree required, CDCA preferred. Or must be able to obtain CDCA within 6 months of hire.
