Working Title: Case Manager- Jail DiversionProgram: Community Support Services- Jail DiversionPay Range:$26.48-29.20 HourlyLocation: The Dalles, Oregon (This position will serve all three counties: Hood River, Wasco, and Sherman County)FTE: 1.0 FTE (37.5 hours per Week)FSLA Status: Not-ExemptDescriptionIntroduction: The Case Manager 1 - Jail Diversion, provides case management services to program assigned clients. This position assists clients in accessing services and collaborates with community partners and agency professionals in case planning and service coordination. This position focuses on supporting individuals engaged with the criminal justice system to prevent recidivism and reduce systemic trauma. The Case manager provides education around the legal system and processes.ResponsibilitiesProvide case management supports (including symptom monitoring, skills training, needs assessments) to Jail Diversion case load.Provide and Collaborates management services in small team setting.Provides Community Restoration duties including Legal Skills Training/case management to .370 Aid & Assist defendants to promote adjudication.Participates in MH Court proceedings, joining community partners to case plan and support justice system involved defendants.Actively participates in meetings to collaborate with community partners including jail, community corrections, housing systems & court systems; services are provided in a variety of community settings, with consideration to client preference.Actively participates in joint case planning and coordination with agency multi-disciplinary team and agency psychiatrists.Assess eligibility of potential clients utilizing agency standards and processes. Assist clients in acquiring services and resources needed to achieve goals and maintain independence within the community. Coordinate with other community partners to develop, provide, and maintain a rich array of services, resources and supports for clients. Provide transportation for clients when required.Meet minimum client service productivity requirements as may be set by the Agency to meet requirements of outside funding and/or reimbursement sources.Utilize agency clinical software system to prepare current client records and reports as necessary and required. Maintain timely and cogent progress notes and plans. Ensure that client records and other documentation are up-to-date and justify the programs case management services provided.Adhere to all clinical and MCCFL program policies and procedures. Remain updated on all applicable rules and OARs.Participate in all team and agency meetings as scheduled. Attend local, regional, and/or state meetings as requested by Supervisor/Program Manager. Within the scope of the position, perform duties independently in the community and promote a positive image of the agency.Depending upon program, this classification may provide after-hours emergency services on a 24-hour, rotating basis, such as telephone consultation, crisis intervention, and pre-hospital screening examinations if qualifications for this service are met.This position may require travel between both office sites and multiple locations in the community. This Job is typically a Mon-Fri 8:30 to 5pm schedule; however, with the nature of the work performed your schedule may vary slightly, as approved, to meet client need. Depending upon location, this classification may provide some administrative support to the team. This may include tasks such as opening and closing case files, greeting visitors and clients, screening telephone calls, etc. Such duties may be required as an essential responsibility based upon budget and/or staffing levels at specific locations.Regular and reliable attendance; professional communication with all contacts; confidentiality.QualificationsIncumbents in this classification must meet requirements as a Qualified Mental Health Associate (QMHA) and certified as such by the Mental Health and Addiction Certification Board of Oregon (MHACBO). Any combination of experience and training equivalent to the following is qualifying:Bachelor’s degree in social work, psychology or related human services field AND two (2) years’ responsible experience in human services; ORHigh school diploma and five (5) years of equivalent training and work experience in human services.Experience working with the severe and persistent mentally ill population is preferred.Bilingual Spanish/English is preferred and may be required for some positions.Must pass all criminal history check requirements as required by ORS 181.536-181.537 and in accordance with OAR 407-007-XXXX to 407-007-XXXX.Must possess a valid Oregon or Washington Driver’s license, maintain an acceptable driving record, and be insurable for client service purposes and for travel between business offices, meetings and/or trainings. The successful applicant must be able to be approved to provide transportation for clients using Agency vehicles.Other Required Knowledge, Skills, and AbilitiesKnowledge: Knowledge of the principles and practices associated with social service case management; knowledge of mental health assessment, treatment and service terminology; knowledge of support services planning; knowledge of symptoms and challenges faced by people with mental illness; knowledge of symptoms and challenges faced by adults and children with severe emotional disturbances including Severe Persistent Mental Illness; knowledge of the interconnectedness between substance use and mental health knowledge of OARs pertaining to the mental health system; knowledge of community resources available to assigned consumers.Skill: Strong assessment skills; skill in the use of medical software; skill in the use of standard office software, keyboarding.Ability: Ability to understand and effectively apply the concepts and practices of a trauma-informed philosophy in performing the work. Ability to work within strong professional ethics and rules of confidentiality to provide effective case management services. Ability to understand and apply mental health case management concepts; ability to implement skills development strategies; ability to identify, implement and coordinate the services in a service plan; ability to meet minimum client service productivity requirements as may be set by the Agency; ability to work collaboratively with other professionals and consumers in the development of service plans; ability to write and maintain timely and cogent progress notes, reports and other professional documents; ability to maintain a strong service orientation; ability to organize and effectively execute work under the pressure of meeting time-sensitive deadlines; ability to work with grace and good humor under pressure; ability to work cooperatively with other departments; ability to communicate effectively, both orally and in writing, and within Sanctuary Model guidelines; ability to establish and maintain harmonious work relationships with co-workers at all levels of the organization, consultants, contractors, community partners, and consumers.Leadership Received:The Case Manager works under the leadership of the Jail Diversion Clinical Supervisor and Community Support Services Manager. The designated supervision emphasizes independence of method and process while indicating accountability for results. Leadership will include triage with staff, guidance and support of clients while demonstrating initiative and responding to concerns.Leadership Exercised:Providing supervisory leadership to other classifications is not a normal responsibility of this position.Physical Demands:The Case Manager must have the ability to quickly assess and respond to concerns related to interpersonal and environmental safety as well as client care. While performing the essential duties of this position, the employee is regularly required to use office automation including computer and phone system that requires fine manipulation, grasping, typing and reaching. The employee may be required to sit or stand for several hours at a time, depending on environment and need. This position may occasionally be required to lift up to twenty (20) pounds, and to kneel, reach, bend or squat while in the office or in the community. The employee should possess a valid driver's license for vehicle travel between business offices in order to attend required meetings and/or trainings. Additionally this employee must be approved to provide transportation for clients using agency vehicles.Work Environment:Work is performed primarily in community based but occurs in various settings, including office environment, community settings and other agency stakeholder facilities. The office environment is typically with moderate stimuli and may often result in exposure to increased sensory experiences. Community settings, such as residence, schools, police precincts, respite facilities, hospitals, courtrooms, probation offices and other public spaces may pose additional risks or discomforts which require safety precautions.The employee should possess a valid driver's license for vehicle travel between business offices in order to attend required meetings and/or trainings. (An alternate mode of reliable transportation may be acceptable.)
