Catholic Charities in Buffalo is hiring a Full-time (35 hours/week) CASE MANAGER for our Immigrant / Refugee Assistance Resettlement Program. 35-hour work week 14 paid holidays for full-time 25 PTO days a year for full-time flexible schedules comprehensive health, dental & vision coverage flexible spending accounts no cost Employee Assistance Program (EAP) 403B Retirement plan with employer contributions training and development opportunities CASE MANAGER FOR REFUGEES JOB SUMMARY: Responsible for coordinating service provision to individuals and families enrolled in program. Provides services directly or links clients with other service providers. ESSENTIAL FUNCTIONS: Determines and documents eligibility for services. Conducts individual and family needs assessments. Develops family self-sufficiency plans. Assists in the implementation of the plan. Provides direct service delivery. Provides appropriate referrals/linkages. Coordinates service delivery. Develops and updates service strategy plans. Provides counseling in personal, family, financial or legal matters. Documents activities per contract requirements. Prepares reports required by funding sources. Inputs data in appropriate database(s). Collaborates with other community agencies. EDUCATION: BA/BS or Associates Degree with 2 years relevant experience or secondary education from an educational institution outside of the US plus 3 years relevant experience. Bi-lingual ability a plus. EXPERIENCE: Must be able to work in a multi-ethnic, multi-cultural setting. Able to function as part of a team working with employment counselors, English as a Second Language (ESL) instructors and interpreters/translators. Must be reliable yet flexible. Must possess a valid NYS Driver's License and vehicle. Evening and weekend availability required. Computer Skills including Microsoft Word required. Must be willing to transport clients in your vehicle.
