Breast Health Nurse Navigator New Hyde Park, NY New York Breast Health , a prominent and respected Breast Health group, is seeking a Breast Health Nurse Navigator to join a well-established and growing pure sub-specialty practice with academic affiliation. NYBH is the leading surgical oncology group specializing in diseases of the breast. In this role you will: Work with the interdisciplinary team to execute a customized plan of care for each unique patient. Guide the patient through the treatment process, reviewing instructions, and scheduling appointments. Facilitate interactions between patients and their physician and referring physician(s) as appropriate. Navigate the patient and family across care settings according to the established patient care plan and clinical pathways Utilize effective communication to ensure patient and caregiver(s) have appropriate information, understand, and have the capacity and tools to adhere to the individualized treatment plan. Contribute to quality-driven performance improvement initiatives. We require: Bachelor’s Degree (Nursing) Current New York State Registered Professional Nursing License Excellent communication (both oral and written), interpersonal, and organizational skills. Demonstrate consistent professional conduct and meticulous attention to detail. Proficiency in Electronic Health Record documentation Minimum of two years’ experience in an outpatient medical office setting. Healthcare or Customer Service Experience Patient Navigation Certification (NYCBS will fully support our team member(s) in achieving and maintaining specialized certification.) Hours: Monday–Friday Salary up to $55/hr. based on experience.
