Summary: Under the supervision of the Director of Brain Health, the BHSRC serves as the primary contact person for all incoming referrals/requests for services (transfer of care, hospital follow-up), scheduling for existing patients (messages, monitoring and managing provider daily schedules), and managing walk-in/same day add on appointments, while making follow-up appointments, creating charts, and indexing paperwork as needed. Essential Duties : Essential job duties for this position include the following items and be provided in-person (clinic). Other duties must be performed as assigned or required. Incoming Referrals: Explain integrated services (active involvement in both medical and BH care), including what it means to be an active BH patient as outlined in policy #80-03. Screen for the desired services, reason for the request, and the referral source to determine potential eligibility (scope of practice, current status with CCHC) as outlined in policy #41-27. Based on screening results, either refer to an external provider (not accept at CCHC), schedule with the appropriate provider, and/or transfer to BH RN or appropriate care team to triage immediate needs and/or questions. Coordinate with the medical scheduler to schedule individuals that want to establish medical care with CCHC as part of our integrated approach. Incoming Referral Coordination/Tracking: Send initial intake paperwork (as needed) and obtain/upload required insurance, financial, and identification information (coordinate with PSR's as needed). Track incoming requests for services (transferred from outside providers), including incoming records from BH and medical providers (if applicable). Follow up with referrals to identify eligibility, status of incoming referral paperwork, and/or schedule with the appropriate provider(s) by maintaining/reviewing the BH fax folder. Patient/Referral Contact: Incoming phone calls; answer calls in a prompt, polite, and helpful manner. Treat all individuals as valued. Return phone messages (to patients/referral sources) within 4 business hours of receiving the message or within 1 business day for messages received 4 hours or less from the end of the business day. Assist individuals walking in without a scheduled appointment to determine needs and coordinate with the appropriate BH staff to provide the needed intervention. Obtain any paperwork needed, including, but not limited to, intake paperwork, proof of insurance, identification, and proof of income. Scheduling: Review patient eligibility for services based on available providers scope of practice and required paperwork on file (intake, incoming records, financial, insurance, etc.) Contact patients and schedule them with the appropriate provider based on eligibility (including assisting with establishing medical services, if applicable). Educate patients and discuss attendance expectations in accordance with policy #70-02. Review and manage provider schedules (daily) to assist with patient access by adjusting schedules (as needed) to reflect real-time access, filling open appointment slots, and assisting with reminder calls as needed. Cancel, reschedule, and schedule follow-up appointments as needed. Environment/Behavior: Perform all functions according to CCHC's mission/vision/core values, established policy/procedure, and confidentiality standards (HIPAA). Treat all employees with respect and communicate using various methods of communication (email, Teams, phone, in person). Maintain a clean, organized, and safe environment for others, including safeguards related to protected health information. Engage in ongoing staff development and clinic wide trainings, including training additional staff on BH related scheduling and incoming referral coordination, as assigned by the Director of BH. Expectations: Report to work as scheduled and actively communicate any needs or concerns with the Director of BH. Actively engage in service delivery (scheduling) and coordination of care with all members of the patients integrated care team (internal or external). Ability to work in person and/or remotely based on department and clinic needs (flexibility). Maintain established policy/procedure, workflows/procedures, and confidentiality of CCHC's patients and employees. Assist with achieving and/or maintaining quality metrics (wait times), performance indicators (visits), and UDS measures. Contribute to the success of the clinic by promoting CCHC's mission, vision, and culture (teamwork, openness, respect). Qualifications: To perform this job successfully, the individual in this position must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the minimum knowledge, skills and ability required. Education/Experience: Minimum of high school diploma or GED required (Associates or higher education with an emphasis on health education or related field preferred). A minimum of two years' experience in scheduling, care coordination, customer service, social service, or community health preferred. Experience in a medical, dental, brain health, or other type of health care or social service related field preferred. Pre-employment drug testing, physical, review of required immunizations including COVID Vaccination and background check required. EOE
