Position Description The Blended Case Manager provides care coordination across multiple life domains on behalf of people receiving mental health services in support of their process of recovery. Minimum Requirements : Bachelor's degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education; or, Registered nurse; or, A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences and 2 years' experience in direct contact with mental health consumers; or, A high school diploma and 5 years of mental health direct care experience in public or private human services with employment as a case management staff person prior to April 1, 1989. ***Mental health direct care experience is working directly with individuals who have a primary diagnosis of a serious and persistent mental illness providing services in a mental health facility or in a facility or program that is publicly funded to provide services to mental health consumers, or in a nursing home, a juvenile justice agency, or a children and adolescent service agency. ***Education credit transcripts are mandatory Applicant must have a reliable vehicle and be willing to utilize it on a regular basis Essential Job Responsibilities Manage a caseload of approximately 20 people Maintain positive working relationship with individuals supporting and advocating for their identified goals and needs C oordina te care across all life domains Work with professional from multiple agencies and systems Develop, implement and review of service plans in collaboration with service recipients and family members/ friends utilizing shared decision making tools and techniques Accurately complete and submit paperwork in a timely manner according to Federal, State and MCO regulations and internal billing requirements Meet productivity expectations with the majority of work performed in the community as billable activity Participat e in regular program, team and individual supervisory meetings and mandatory agency training Participa te in daytime, evening and weekend on-call responsibilities A ssist consumers as needed in carrying groceries, moving, pushing and/or lifting wheelchairs or other physical activities Adhere to all laws related to Medicaid Fraud, Waste and Abuse and compliance requirements Participate and advocate at court hearings i.e. County or Federal Court, District Justice, Social Security, Child Welfare, Housing
