The Health and Social Services Manager will lead a care management team while overseeing day-to-day clinical operations and health plan clinical initiatives to support achieving quality and affordability targets. The HSS Managers will support a team within different regions in Northern Indiana. Responsibilities will include process improvement, execution of clinical decision processes, and team management, to include performance management and implementation of program initiatives. If you reside in Indiana, you will have the flexibility to work remotely* as you take on some tough challenges. Primary Responsibilities: Selects, manages, develops, mentors and supports staff in designated department or regionDevelops clear goals and objectives for performance management and effectively communicates expectations, and holds the team accountable for resultsIn order to meet the unique needs of our members, have an intimate understanding of the contractual requirementsIdentify, select, structure, and prioritize process improvement projects, ultimately implementing changes to meet program requirementsConducts monthly audits of staff and participates in all other required audits and business monitoringEnsures standardized execution of workflow processes, including conducting performance audits, quality reviews, and compliance adherenceParticipates in training and coaching of direct reports as neededCollaborates across Optum and UHG and interacted with Medical Directors, Senior Leaders, Quality, and other stakeholders You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. Required Qualifications: Active and unrestricted RN license in the state of Indiana or Master's level social worker5+ years of experience in providing care coordination to older adultsExperience working within the community health setting in a health care roleExperience facilitating and working in coordination with an Interdisciplinary TeamExperience coaching or mentoring staffIntermediate level of experience with Microsoft Word and ExcelProven ability to navigate a TEAMs environmentResident of Indiana Preferred Qualifications: 1+ years management experienceCase Management experienceExperience with Long Term Service and Supports or Waivers ProgramBackground in managed care *All employees working remotely will be required to adhere to UnitedHealth Group's Telecommuter Policy At UnitedHealth Group, our mission is to help people live healthier lives and make the health system work better for everyone.
