Patients and their families know us for our personalized, high-quality care, delivered with empathy, kindness, and respect TITLE: After Hours Admission/Visit Registered Nurse (RN) OFFICE LOCATION: South San Francisco PATIENT TERRITORY: San Mateo - San Francisco SCHEDULE: Per Diem or Part Time SHIFT: 4pm-12:30amThe posted compensation range of $56.95 - $78.30/Hour is a reasonable estimate that extends from the lowest to the highest pay Pathways Home Health & Hospice in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. Pathways Home Health & Hospice may ultimately pay more or less than the posted range as permitted by law. POSITION SUMMARY: The Shared Services After Hours Visit RN is responsible for providing skilled nursing visits to patients and their families as requested by Intella Triage. Documents observations and interventions related to the patient’s environment; physical, mental, emotional and social status. Coordinates care with othermembers of the Interdisciplinary Group (IDG) or team, including the patient’s physician. AREAS OF RESPONSIBILITY: ●Admits patients to service, assesses the patient’s and family’s physical, psychosocial,environmental, safety and developmental needs. Develops an individualized plan of care basedon the assessment.● Obtains informed consent from patients and families and assists in completing forms requiredfor hospice admission per regulations.● Assesses patient, provides and documents appropriate skilled nursing care, including teaching,family support and other actions as needed.● Assesses patient’s environment; physical mental and social status and eligibility for services inaccordance with third party payee regulations.● Coordinates care with the patient’s case manager, informing him/her of significant changes inthe patient’s condition and needs. Documents these communications.● Contacts patient’s physician when necessary.● Demonstrates all skills of a registered nurse, including vein puncture and management of highinterventions in the home, e.g. IV’s.● Maintains in-dated and adequate stock of medical supplies and agency forms.● Knowledgeably discusses overall history, philosophy and services of Pathways Home Healthand Hospice.● Admits patients to service, assesses the patient’s and family’s physical, psychosocial,environmental, safety and developmental needs. Develops an individualized plan of care basedon the assessment.● Obtains informed consent from patients and families and assists in completing forms requiredfor Hospice and Home Health admission per regulations.● Provides patients and their families with guidance regarding health care decision-making andoptions for making advance directives in coordination with the social worker.● Assesses all aspects of the patient’s pain and other symptoms and develops an individualizedsymptom management plan.● Anticipates, prevents and treats undesirable symptoms or secondary symptoms including use ofnon-pharmacologic measures.● Supports, instructs and educates the patient, family, caregivers, and facility staff.● Documents nursing assessments, problems, appropriate goals, care interventions, patient andfamily outcomes, and evaluation of these in the electronic record.● Accesses electronic device during patient visit and documents during or immediately after thevisit or per regulation.● Reports regularly to Shared Services After Hours Triage RN, as appropriate, and RN CaseManager or designee regarding care provided, and when patient condition is found to beunstable and there is a need for changes in current plan of care.● Supports, instructs and educates the patient, family, caregivers, and facility staff.● Attends educational in-service trainings, and other agency meetings as requested by manager.● Maintains the dignity of the dying patient and supports the patient’s and family’s uniquespiritual and cultural beliefs.● Provides holistic, family-centered care across the treatment settings to improve the quality oflife.● Knowledgeably discusses and initiates non-pharmacologic interventions as appropriate topromote comfort, improve quality of life, and help manage emotional, spiritual, and physicaldistress.● Participates in the hospice program’s quality assessment performance improvement programand in-service education related to areas of practice as appropriate.● Meets productivity standards as established by the manager.● When in a facility, adheres to “Pathways Standards of Care and Practice in Facilities” orequivalent document.● Prints out and files visit documentation in facility (RCFE or SNF) binder or chart per facilitypreference at the time of visit.● Maintains timely communication with patient, family, and facility staff regarding all aspects ofpatient care as appropriate.● Demonstrates awareness of and adherence to the safety, infection control and other policies ofthe facility.● Maintains and updates level of knowledge and skills required to provide quality nursing care.● Creates, documents, and maintains the medication list. Evaluates medications for side effectsor interactions and communicates these.● Effectively instructs patient, family, and/or staff use of medications and reconciles medicationrecords.● Effectively communicates with and uses the Hospice prescription plans, formulary/preferreddrug list, and general hospice medication coverage philosophy.● Adheres to all agency policies and procedures, including updates, as communicated in written,spoken, and electronic forms.● Promotes a culture of service when representing Pathways by at all times treating internal andexternal contacts with respect, remaining professional, communicating clearly, preventingconflict, following through on commitments, and actively listening. Positively promotesPathways by offering assistance and information.● Assists in development of Pathways protocols, procedures and policies as requested.● Demonstrates all skills of a Registered Nurse, including those skills required by the agencythrough competency testing.● Performs other duties as assigned consistent with skills and training and the mission and goalsof Pathways. QUALIFICATIONS: ● California RN license. Hospice or Home Health experience preferred● Graduate of an NLN accredited nursing school.● Must be highly motivated, self directed, flexible, and able to work well and participate in amultidisciplinary team setting. Excellent nursing assessment, communication, andproblem-solving skills.● Minimum of one-year experience as a professional nurse within the last three years or abaccalaureate degree in nursing from a program accredited by the National League for Nursing● California driver’s license and automobile insurance with minimum Bodily Injury Liabilitycoverage Insurance per agency policy.● Current CPR Certificate– American Heart Association “BLS Health Care Provider” requestedupon renewal.Powered by JazzHR
