Position Summary Working as a member of the interdisciplinary Community Wellness team, the Social Worker provides a diverse range of client care to ensure client engagement and to reduce or remove barriers. This care includes extended community outreach, health education, case management, treatment support, counseling, group facilitation, community programming, and advocacy for clients with acute and complex care needs. The Social Worker provides this care to individuals while promoting wellness, disease prevention, and chronic disease management and adopting a long-term approach to client care consistent with the philosophy of harm-reduction. Qualifications Must have a demonstrated understanding and grounding in Indigenous ways of knowing, doing, and being along with their application to social work practice.
