Our team of healthcare professionals positively impacts the health, well-being and quality of life for those we serve providing comprehensive, integrated, affordable healthcare and education to patients. We utilize a team-based model including embedded Behavioral Health Clinicians as well as Case Managers, Care Coordinators and Community Health Workers. The Manager of Behavioral Health is responsible for the delivery and oversight of Behavioral Health services in Amoskeag Healths four clinics. The Manager works to support a team-based, integrated Patient Centered Medical Home model of care as outlined by the National Committee on Quality Assurance (NCQA). Through the implementation of evidence-based treatment practices, mission-driven goal setting, personal and team accountability, the Manager leads the delivery of high-quality behavioral health and social services in our diverse and vibrant Federally Qualified Health Center.
