SCOPE OF ROLE Reporting to the Supervisor, Intake Services and Director, Diagnostic Assessment and School Based Services, the Counselor, Diagnostic Assessment will provide outstanding assessment services to clients and their families. The Counselor, Diagnostic Assessment will be responsible to make appropriate decisions related to client treatment that includes diagnosis, level of care , program/services referrals and recommendations; and provide walk in counseling services as needed. HOW YOU’LL SUCCEED: * Conduct exceptional assessment and diagnosis services for clients including gathering information from and coordinating care with collateral sources, with appropriate releases of information. * Assist with obtaining signatures, consents, and review and refer patients to financial representatives as needed based on insurance. * Maintain client charting in accordance with Medicaid, JCAHO, ODADAS, ODMH, CARF and professional standards. * Point of contact until clients are established with recommended services by providing follow up to patients and collaterals as needed. * Participate in peer review. * Obtain ROI’s and required medical records. * Actively consults with supervisor to act upon ethical and legal aspects of the organization. * Acts to understand and safeguard the welfare of clients. * Other duties as assigned. KNOWLEDGE & EXPERIENCE: * Bachelor’s Degree required. Masters preferred. * Valid unencumbered LSW, LISW, LPC, LPCC, LIMFT or MFT License in the state of Ohio required. * 1-3+ years’ of experience in the Behavioral Healthcare field required. * American Heart Association (AHA) Basic Life Support (BLS) certification required. * Possess a working knowledge of mental illness and the disease concept of chemical dependency, Mental Health and Chemical Dependency treatment, Alcoholics Anonymous philosophy, and group dynamics. * Experience collaborating with other team members to optimize outcomes clients. * Familiarity of state and federal healthcare regulations. * Awareness of community and state support resources for population served. WORKING CONDITIONS: * Work is normally performed in a typical interior/office/clinical work environment. * While hours of operation are generally standard, flexibility to work evenings and extended hours may be required. * Requires periods of sitting, standing, telephone, and computer work. * Hearing: adequate to hear clients or patients in person, over the telephone or through telehealth technology. * Speaking: adequate to speak to clients or patients in person, over the telephone or through telehealth technology. * Vision: Visual acuity adequate to perform job duties, including reading information from printed sources and computer screens. * Physical effort required: occasional lifting and carrying items weighing up to 15 pounds, unassisted. * Possible exposure to blood borne pathogens while performing job duties. * Frequent bending, reaching, and repetitive hand movements, standing, walking, squatting and sitting, with some lifting, pushing and pulling exerted regularly throughout a regular workday. * Sufficient dexterity to operate a PC and other office equipment.
