Registered Nurse with background in Mental Health, Behavioral Health, Psychiatric$70,000-$80,000 with $5,000 sign on bonus Position Purpose: The residential RN provides direct nursing care within the RN scope of practice as defined by the VT Nurse Practice Act and manages medical related documentation for residents in HCRS Adult Services Residential programs under the supervision and direction of the Nurse Manager. The RN completes nursing related assessments and Nursing Care Plans, implements treatments, and interfaces with other medical care providers to coordinate services. The residential RN supports residents to develop independent skills in accessing medical care and self-determination in treatment approaches. The RN is part of the leadership team of the residential programs and provides direction for staff and residents in the implementation of health and wellness practices. The RN trains and supports non-licensed medication delegated Residential Specialist staff to follow HCRS and program policies and procedures. Principal Responsibilities and Duties: The RN is responsible for overseeing that residents’ medication orders are accurate and up to date, and that medications are correct and available. Ensure that safe practices are followed for medication storage and administration. Maintain a clean and organized work space. Oversee that medical supplies and tools needed for resident care are up to date, in good working order, stored safely and easily accessible. Complete Nursing assessments accurately and on time. Review medication related documentation for accuracy and completion at TCR and Level 3 community care homes. Complete Nursing Care Plans. Implement care plans and document on progress. Document changes and progress in the electronic health record through nursing shift notes, treatment charts. Document accurately in medication administration records. Monitor for any changes in physical or mental status and adjust interventions as determined in conjunction with treatment team and/or MD direction. Provide Nursing oversight of residents in level 3 residential care homes as part of the larger nursing care team which includes LPNs and med delegated staff. Provide timely and accurate updates of resident progress and facilitate communication of resident needs and concerns to support the HCRS psychiatrist and other care providers within and outside of HCRS. Promote respectful and compassionate communication with residents and staff. Advocate for self-determination and full participation in all areas of care within the residential programs and with external providers. Coordinate medical care as needed with the goal of facilitating skills needed for residents to make their own appointments and follow through with care. RN is responsible for ensuring follow up care is available and documented. Provide explanation of medical reports/plans to residents and to other staff, within scope of practice. Communicate with Residential Leadership Team if resident needs are not being met. RN is a mandated reporter, per VT Nurse Practice Act. Provide emergency medical care as needed and/or as directed by MD, or resident’s health care provider. May attend medical/other appointments with residents as needed. Administer injections as ordered by the physician. Participates in a nursing on-call rotation, that provides direction and support to the HCRS community Scope of Position: Residential RN provides additional nursing related support to other HCRS residential programs. Reports to the Nursing Manager. This is a Union position. Key Knowledge, Skills and Abilities to successfully perform this job: Skills necessary to provide nursing care at the level of a registered nurse in a Level III Community Care Home Understand the of the scope of practice of an RN as defined by the Nurse Practice Act Attention to detail in following procedures related to accurate implementation and documentation of medication orders and treatments Accuracy in medication administration records and progress notes. Ability to use a computer and navigate an Electronic Medical Record Ability to understand complex written orders and follow written procedures and protocols Ability to train and delegate staff Ability to communicate complex medical related information to individuals with vary in levels of medical acumen Ability to perform and complete nursing assessments and develop nursing plan of care Ability to develop trust and rapport with people who may be experiencing intense emotional states Ability to take a leadership role in providing quality compassionate care, modeling and maintaining professional boundaries, and understanding and following agency policies and program procedures Ability and willingness to learn new skills as needed and adjust to changes in a fast paced working environment Ability to establish and maintain good working relationships with staff within the agency and with external providers, such as physicians’ offices, hospitals, and pharmacies Relationships: Reports to Nurse Manager Works closely with Residential Coordinators, Residential Shift Leaders, Residential Specialists, HCRS psychiatrist, other HCRS, Residential RN and LPNs Supports HCRS psychiatrists to provide psychiatric care for residents Interacts regularly with external providers and pharmacies to coordinate care Qualification Statements Minimum: Must have a current State of Vermont RN license in good standing. Solid working knowledge of and compliance with the Nurse Practice Act. Excellent organizational, interpersonal, communication, and listening skills. Must have First Aid and CPR certification. CPI Certification (may obtain after hire). Willingness and ability to meet flexible work schedule. Possession of a valid driver’s license, car insurance and reliable transportation. May be required to transport residents Preferred: Mental health experience preferred. Two years’ experience in any medical setting Working Conditions and Additional Information: While performing the duties of this job, the employee is regularly required to sit, walk, climb stairs, stand, talk, and hear. May be exposed to traumatic situations with potential for physical and emotional harm The employee may occasionally lift and/or move up to 50 pounds. Specific vision abilities required by this job include close vision and the ability to adjust focus. The employee requires sufficient manual dexterity to perform nursing functions (medication preparations, injections, glucometer tests). The work takes place in residential setting.
