In this role, you will provide high-quality care to our patients on an as-needed basis. This position is ideal for a nursing professional looking for flexible work opportunities while contributing to a dynamic and patient-focused environment. Key Responsibilities: Monitor patient status and report any changes to the healthcare team. Educate patients and families on health conditions, treatment plans, and post-discharge care. Collaborate with a multidisciplinary team to achieve the best patient outcomes. Maintain accurate and thorough patient records. Complete monthly assessments and advise direct support professionals Travel to individual’s home Qualifications: Current and valid RN license in the state of Indiana. Bachelor of Science in Nursing (BSN) preferred. Minimum of 1 year of nursing experience BLS/ACLS certification required. Strong clinical skills and the ability to make decisions in a fast-paced environment. Excellent communication and interpersonal skills.
