Physical Therapist (PT) Overview:Join a leader in the home health care industry that provides nursing oversight on each and every case - no matter how small. If providing compassionate, high quality care as a Physical Therapist is your passion, BrightStar Care has an opportunity for you!Physical Therapist (PT) Responsibilities:Provide therapy services to clients according to a physician's written orders. This may include, but is not limited to the following: Assess and evaluateDevelop a plan of care and treatment making sure plan of care is consistent with assessment Participate in the development of the client's total plan of treatment Assess the home environment especially as it relates to fall risk and interventions and home safety Assess for muscle strength, mobility, gait, and range of motionAssess for potential for rehabilitationReport client's response to treatment or changes in condition to the Director of Nursing and physician Initiate therapy program and instructs other personnel and/or family members and caregivers in specific phases of therapy with which they may assist with the client care. Participate in client case conferences. Teach/educate client in the use of prosthetic device, as required. Assist in identifying client and family/caregiver needs for other home health services and reports these needs to the Director of Nursing. Prepare and submit clinical and progress summaries based on identified and attained goals. A copy of the summaries will be submitted to the Director of Nursing. Participate in discharge planning, as appropriate. Provide in-service education for agency personnel, as needed. Supervise therapy assistants, if needed, in accordance with organization policy and state regulations. Assure compliance with all local, state and federal laws and regulations. Physical Therapist (PT) Qualifications:Master's Degree in therapy from an accredited therapy program. Passed a National Therapy Examination and/or state examination as required by the state regulation.Compliant with continuing education, as required in Arizona as a condition of maintaining licensure.Comply with Arizona state mandated Therapy Practice Act and other legal mandatesLicensed or certified to practice as a therapist as required by Arizona.. Two (2) years of appropriate experience as a therapist.
