Band 6 Social Worker Gloucester Health and Care NHS Trust Coliers Court, Cinderford , GL14 2QA Monday to Friday ASAP Start – Ongoing Contract £35ph Job Role To work as a Care Co-ordinator or Lead Professional where applicable with individuals who have serious and enduring mental health difficulties. To develop and provide evidence based programmes of care for service users of working age with mental health problems who are receiving treatment from the Recovery Team, with a focus on social care To ensure a clear focus on recovery and social inclusion. To support and supervise both registered and non-registered staff in delivering care. The role will entail, backssment , support planning and review of social care packages in robust and timely manner, in accordance with The Care Act 2014 requirements Job Responsibilities Manage the service provision for a defined caseload of service users. To act as Care Manager where appropriate To act as care co-ordinator in line with GHC Trust policy for Care Programme Approach To ensure appropriate risk backssments are completed To ensure all child protection, adult safeguarding and Mental Capacity Act issues are dealt with according to policy To respond to requests for backssments and review of need working in partnership with user, carers and other professionals as agreed within the multi-disciplinary team and with the Team Manager, including out of county/area reviews To develop and deliver strengths based packages of care including specific psychosocial interventions where appropriate To delegate the delivery of specific interventions to support workers as appropriate To work in partnership with service users, carers,.
