What does a Social Worker do at Hillingdon? Hillingdon Adult Social Care has three locality teams working with adults with a physical disability and older peoples. The team responds to safeguarding enquiries, carries out best interest backssments for service users, implementing and reviewing care packages in the community (Service users' homes / Supported Living / Care Homes). The successful applicant will work closely with the CCG, GP's, Community Health teams and voluntary sector agencies. We are looking for an enthusiastic social worker with skills and knowledge in the Adult Social care field, focusing on person centred and strength-based approach in social care need backssment, support planning, mental capacity backssment and best interest decision making, S42 enquiry and Do LS challenges.
