The Role : Complex Needs Coordinator: As a Complex Needs Coordinator, you will play a crucial role in our integrated healthcare framework, providing essential support to individuals aged 18 and over who are dealing with drug and alcohol dependencies. Our services are designed to deliver Psychosocial Drug and Alcohol Interventions for service users identified as having substance abuse issues. Services will deliver client centred treatment using both harm reduction and abstinence: based treatment approaches depending on the requirements of the individual client. You will be required to manage clients who have been referred from hospital services and from partner agencies. Services provided to them will be integrated within the substance misuse service to provide appropriate harm reduction advice and guidance. You will be required to backss, care plan; deliver structured 1:1 and group: work sessions including structured treatment programmes. Responsibilities : Complex Needs Coordinator: Conduct comprehensive backssments, including initial and risk backssments, to inform individualized treatment plans. Carry out interventions tailored.
