The Role – Mental Wellbeing Practitioner HMP Chelmsford: The Mental Wellbeing Diagnosis Practitioner will operate across HMP Chelmsford, working closely with adults either in a group face-to-face setting or on a one-to-one basis helping people who are experiencing mental health and substance misuse issues. As a Mental Wellbeing Practitioner you’ll be required to manage a caseload, provide advice and guidance, backss, care plan and refer where applicable. You may be asked to attend meetings with our multi-agency partners, to represent Forward Trust and report back on service updates. Key Responsibilities – Mental Wellbeing Practitioner: As a Mental Wellbeing Practitioner, your responsibilities will include: Conduct backssments to evaluate risk factors and inform mental wellbeing treatment plans for individuals.
