JOB SUMMARY: The Occupational Therapist is primarily responsible for delivering outpatient rehabilitation services to older adults in their homes. This includes private residences or rooms within independent living facilities. With Powerback Rehabilitation to You, the Occupational Therapist conducts assessments and treatments in the most functional environment the patient's home. They design and implement occupational therapy programs aimed at facilitating rehabilitation, restoring, reinforcing, and enhancing performance. The focus is on developing essential skills for adaptation and productivity to mitigate or correct pathology. This role offers a flexible schedule and diverse work environments. RESPONSIBILITIES: Supervise Occupational Therapy Assistants, temporary licensees, students, and support personnel as needed, following state licensure requirements and professional standards.Ensure adherence to departmental and patient treatment schedules, timely clinical and billing documentation, and attend relevant meetings.Participate in patient care and staffing conferences to contribute to treatment plans.Make referrals to other disciplines or agencies to address comprehensive patient needs.Collaborate with the rehabilitation team, including community physicians, to determine appropriate therapeutic regimens.Adapt programs or care plans based on individual patient needs.Maintain a safe environment by identifying and eliminating hazards.Order necessary supplies and equipment and ensure their cleanliness and maintenance.Prioritize patient service, ensuring high-quality care in a compassionate atmosphere.Screen, examine, and evaluate patients, establish diagnoses, and develop treatment plans in collaboration with patients and caregivers.Implement occupational therapy treatment plans, including therapeutic techniques in various areas such as cognitive skills, muscle strength, mobility, and activities of daily living.Design, fabricate, fit, and train patients in the use of adaptive devices.Select appropriate activities based on patients' capacities and interests.Instruct patients in functional activity participation and evaluate progress.Conduct routine reexaminations to modify treatment plans as needed.Perform other duties as required.
