CINQCARE Field/Virtual Nurse Practitioner Position Description Overview The Field Nurse Practitioner reports to the Chief Medical Officer, ACO REACH , with accountability for providing clinical judgment, organization, and evidenced-based analysis in keeping with the CINQCARE model of care. The Nurse Practitioner should embody CINQCARE’s core values, including Imagination , Trusted, Empathetic, Committed, Creative, Humble and Community-Minded . At CINQCARE , we don’t have patients or customers – we have Family Members. Primary Responsibilities: The Field Nurse Practitioner (NP) will have the following responsibilities: Provide general medical care to various and/or specific patient levels – adults, geriatrics, and pediatrics. Perform comprehensive evaluations including history and physical exams for risk adjustment and other regulatory assessments. Address both chronic and acute primary care complaints, and able to ascertain medical urgency. Order pertinent diagnostic laboratory and radiology testing for the medical diagnosis or presenting symptom; able to work within an environment of limited resources and therefore uses diagnostic tests judiciously and appropriately. Create and implement a medical plan of care. Provides care coordination and preventative care services for geographical as well as virtual targets who are stratified by risk. Provide post discharge coordination to reduce hospital readmission rates and emergency room utilization. Perform face-to-face in-person visits in a variety of settings. Additionally, perform face-to-face synchronous video communications using Tele-Health platforms based on business need and leadership direction. Prescribe medications as appropriate. Perform timely documentation in an electronic medical record computer system or other method of documentation. Engage in practices constituting the practice of medicine in collaboration with and under the medical direction and supervision of a licensed physician to the degree required by state laws. Perform other job-related duties as assigned. General Duties The Field NP will have the following duties: Leadership : The Field NP will lead in defining and executing strategies and solutions to create business value for CINQCARE. Strategy : The Field NP will help establish the business strategy and roadmap: improve outcomes for CINQCARE’s Family Members; enhance the efficacy of the practice. In establishing the business strategy, the NP will define and innovate sustainable revenue models to drive profitability of the Company. Collaboration : The Field NP will ensure that CINQCARE’s capabilities form a cohesive offering, including by working closely with other business divisions to learn their needs, internalize their knowledge, and define solutions to achieve the clinical objectives of CINQCARE model of care. Knowledge : The Field NP will provide clinical subject matter expertise, seek specialty consultation as appropriate and is responsible for knowing when a patient needs physician oversight and when the patient's medical condition may be beyond the scope of knowledge of the NP. Culture : The Field NP is accountable for creating a productive, collaborative, safe and inclusive work environment for their team and as part of the larger Company. Qualifications The Field NP should have the following qualifications: Education: Nursing Degree or Nurse Practitioner Degree from an accredited U.S. school is required. Nurse practitioners must have completed accredited education and training for nursing and nurse practitioners. An active unrestricted medical license in CA is required. DEA Certification required. Additional state licenses are a bonus. Experience: At least 2 years of experience as a Nurse Practitioner. Preferred experience in Health Home programs as a licensed clinician, especially in management of chronic conditions. Experience with underserved populations facing socioeconomic barriers to health care. Fluency in a language in addition to English is a plus. Immunization and phlebotomy skills. The Field Nurse Practitioner will possess the ability to perform certain in-office procedures safely in an outpatient setting. Communication: Excellent verbal, written communication and presentation skills; ability to clearly articulate and present concepts and models in an accessible manner. Relationships: Ability to build and effectively manage relationships with patients, the community business leaders, and external constituents. Culture: Good judgement, impeccable ethics, and a strong team player; desire to succeed and grow in a fast-paced, demanding, and entrepreneurial Compan y. PHYSICAL DEMANDS Requires sitting and standing associated with a normal office environment. Manual dexterity needed using a computer keyboard. While performing duties of this position, the employee is regularly required to talk or listen. The employee needs to be able to see and frequently is required to use hands or finger, handle, or feel objects. The employee is frequently required to stand, walk, sit; and reach with hands and arms. Ability to travel frequently by car and/or public transportation. Work Environment: • Will require regular travel to Physician offices, hospitals, sub-acute facilities, community partners and non-home-based market offices. • The noise level is usually moderate. Compensation: $60-70k part time Location: Hybrid: Mainly a Field position and will make visits to patient homes, hospitals, facilities, CINQCARE practices, CINQCARE main offices, other entities as appropriate.
