Description: POSITION: Admissions NurseREPORTS TO: Intake and Admissions Team LeaderFLSA STATUS: o exempt o non-exemptPOSITION SUMMARY:Admit and provide direct patient care for Hospice patientsEDUCATIONAL QUALIFICATIONS: Current NC license as a registered nurseMinimum 2-3 years of nursing experience requiredActive patient contact or completion of a nursing refresher course within the past 3 yearsCPR certificationKNOWLEDGE AND SKILLS:Knowledge of and commitment to the hospice philosophy of careAbility to work on an interdisciplinary teamAbility to work a flexible scheduleWillingness to work with patients/families regardless of race, color, creed, age or sexual orientation. Must respect their beliefs and philosophy with an attitude of openness and receptivenessRequirements: TRAINING-INITIAL AND ON-GOING:Complete Hospice volunteer/staff training program Complete Hospice orientation programObtain 12 hours of continuing education per year including mandatory safety in-serviceAttend minimum of six staff meetings per yearMaintain CPR certificationDUTIES AND RESPONSIBILITIES:Assist in orienting new team membersParticipate in quality improvement programAdhere to strict confidentialityAdhere to safety policy and practicesComplete employee time sheet accurately per policy Cooperate with all government agencies and outside auditorsAssist with updating policies and procedures as appropriateComplete annual TB screeningAssure all license, certifications, continuing education information and any other required documentation are submitted to human resources and kept currentMaintain up-to-date documentation required and specified in Hospice policy and procedurePerform other duties assigned by supervisorEvaluate the physical, financial, psychological, spiritual, and pre-bereavement needs of the patient/family in coordination with interdisciplinary teamEducate the family about Hospice servicesIf patient is not appropriate, recommend other services available, with referrals to other community agencies as appropriateCommunicate patient/family information to the interdisciplinary teamDevelop plan of care with goals defined by patient/family and Hospice teamDevelop a set of interventions needed to reach goalsPerform the duties of a primary nurse as neededComplete documentation within hours designated in policyCoordinate same day phone calls to new admissions Enter medications into SuncoastAssign primary nurse, home health aides services and other services as directed by supervisorCoordinate durable medical equipment and pharmacy, as needed, for all patients upon admissionIncluded in home care nurse minimal staffing days Serve as primary nurse to patients with life expectancy of 24-48 hoursMaintain small caseload in times of high census Provide support to patient/families as directed by supervisorPHYSICAL QUALIFICATIONS:Speech and hearing skills necessary for telephone interaction and face to face contactVisual acuity to read chart and physician's ordersAbility to stand and walk for long periods of timeManual dexterity to establish IV sites, manipulate tubing, and other patient care suppliesWith assistance, ability to lift and support an average of 150 lbs.Ability to sit as required to perform duties Ability to pull/push, and bend frequentlyAbility to squat, kneel, climb, and balanceSPECIAL REQUIREMENTS:Must have an automobile with required insurance coverage and a current driver's licensePI24fd26cef7fe-25660-34408666
