Acenda's expert team helps each individual, family and community move forward in their journey towards health and happiness.The LifeSet Program is an evidence-informed program model to address the needs of young people who are transitioning out of foster care and need individualized skill development and support to transition successfully into adulthood. The Specialist will be required to work flexible hours, and be on call 24/7 (365 days), when required by the programJob Overview: The Specialist is responsible for providing services to address the needs of young people in their day-to-day livesKey Responsibilities: * Facilitate processes to assist the participants in obtaining and maintaining a safe home, continuing education, independent living, and employment. * Communicates programmatic or job-related concerns to supervisor and makes suggestions for improvement in an appropriate and timely manner. * Submits Level of Service (LOS) within program requirements. * Plays an active role in meetings and trainings designed to enhance agency and program operations. * Must be able to analyze work, set goals, develop plans and utilize time effectively and efficiently.Requirements: * Bachelor's Degree in social services or other related field required and one (1) year of experience working with youth or in the social services field. * The Specialist will be required to work flexible hours, and be on call 24/7 (365 days), when required by the program * Must have and maintain a valid driver's license, use of an insured vehicle and an acceptable driving record.We provide * Competitive salary and 401k with a match * Year-end performance bonuses * Health, vision and dental coverage * Generous number of vacation days each year * CEU's and free training programsAcenda's Equal Employment Opportunity CommitmentAcenda Integrated Health is an equal opportunity employer, and all qualified applicants will receive consideration for employment without regard to race, color, religion, age, sex, national origin, disability status, genetics, protected veteran status, sexual orientation, gender identity or expression, or any other characteristic protected by federal, state or local laws.
