Acenda's expert team helps each individual, family and community move forward in their journey towards health and happiness.Key Responsibilities: Job Overview: As part of a multi-disciplinary, core services team, the PACT Nurse provides nursing assessment, direct support services, and primary health care to individuals with seriously and persistently mentally ill individuals enrolled in the PACT program.
