Forensic Coordinator: Serves as primary clinician/case manager for NGRI consumers released from CMHIP, either designated as Community Placement clients, or Conditionally Released clients. Maintains regular communication and liaison with FCBS team. Complies with all Community Placement and Conditional Release Policy requirements, including completion of all required paperwork. Prepares Quarterly Reports on each client's progress for FCBS Team and the courts. Hospital Liaison: As part of the team of Hospital Liaisons, serves as the key connection between inpatient psychiatric facilities and Jefferson Center. The liaison facilitates information exchange between the inpatient team and Jefferson Center providers. The liaison provides recommendations to the hospital providers about the goals for the hospitalization and development of a comprehensive discharge plan. Ensures appropriate services are in place at the time of discharge within established time frames. Forensic Responsibilities: Provides clinical services to assigned consumers, including ongoing assessment of risk and compliance with terms of release. Prepares and submits timely quarterly reports to Forensic and Community Based Service Team (FCBS) and the District Attorney ( DA). Facilitates communication and coordination between FCBS, the legal system, and Jefferson Center. Incorporates conditions specified by FCBS and the court into Jefferson Center treatment planning process. Facilitates communication and coordination between FCBS and the center. Reports any unusual incidents, special circumstances, violation of conditions, or clinical decompensation to FCBS. Attends quarterly meetings with FCBS team to identify issues and monitor progress. Hospital Liaison Responsibilities: Serve as liaison between Jefferson Center and inpatient treatment teams. Provide case management with clients as needed. Engage in regular communication with the crisis team and the client's Jefferson Center treatment team regarding admissions, progress in 24-hour care, and projected discharge date. Work directly with inpatient providers to determine client's needs while inpatient and anticipated needs at discharge. Participate in staffing, treatment team meetings, family sessions or individual sessions in the facility as clinically indicated. Communicate urgent situations and needs to the Jefferson Center supervisor and the crisis team. Set up discharge/step-down appointments as indicated, including prescriber appointments, and document according to Jefferson Cener guidelines. Essential Duties: Completes all required trainings Attends supervision at times and intervals agreed upon with supervisor Completes all documentation in compliance with Jefferson Center guidelines Maintains a professional attitude and conduct. Maintains effective interpersonal relationships with consumers, other Jefferson Center staff, community partners, and the public. Note: Employees are held accountable for all duties of this job. Education, Knowledge, Skills & Experience Required: Master's degree in counseling, social work, psychology or related field required. 2 years related experience and/or training required. Licensure preferred. Salary Grade 51 - Master's Unlicensed Clinician $47,600 to $60,400* Salary Grade 52 - (Licensed Clinician LCSW, LMFT, LPC) $57,000 to $72,700* Additional Salary Information*: All salaries at Jefferson Center are calculated based on years of total relevant experience.* The salary range above is based on 1.0 FTE (full time equivalent) or 40 hours per week. Less than 40 hours/week will be prorated and adjusted to the appropriate FTE.* Additional pay provided for evening, overnight, and weekend shifts.* Master's Bonus LPC/LMFT Bonus LCSW/LP Bonus $1,500 1 month anniversary$1,500 1 year anniversary($3,000 Total)$1,000 1 month anniversary$1,000 6 month anniversary$1,000 1 year anniversary($3,000 Total)$2,000 1 month anniversary$1,000 6 month anniversary$1,000 1 year anniversary($4,000 Total) Additional Bonus Information*: Based on full time (40 hours/week).
