The Care Coordination Assistant (CCA) partners with Nurse Case Managers and the clinical team to provide support and coordination services. The CCA works closely with families to link them to clinical, community and social resources as necessary. The CCA must demonstrate skills in creative problem-solving, and have knowledge of the assets and challenges of the population they are serving. They will work with medical professionals to ensure follow-up and coordination of recommended medical management.High school education or equivalent.Two years of experience in a related field with an emphasis on customer service. Scheduling experience is preferred.Ability to type 35 words per minute to type forms, letters and prior authorizations and perform data entry.Experience with electronic medical records (Epic) and Microsoft applications including Word and Excel.This is a part time position that comes with our full benefits package.The schedule consists of working every Thursday and Friday and every other Wednesday from 8:00-4:30.Children's Wisconsin is an equal opportunity / affirmative action employer.
