Provides EDTC, hospital and/or community follow-up when an individual is violently injured or victimized and provides crisis intervention, support and education about the effects of violence directly related to their victimization. Schedules and conducts home visits and provides ongoing case management and resource referrals in collaboration with the care team and available community supports. Accountable for helping program participants establish linkages to appropriate youth development and family activities. Works as part of the Project UJIMA team in ongoing program improvements and tracking project outcomes. Bachelor's or Associate degree in social science OR two years of work experience with adult and youth-serving programs, particularly adolescent work experience, and familiarity with community programs and services available in Southeastern Wisconsin. Professional demeanor and excellent communication skills, written and verbal, to relate to people of varying socioeconomic class with differing lifestyles or cultural values and to interact with community based service organization professionals. Experience working with families of diverse ethnic, cultural and socioeconomic backgrounds strongly preferred. Working knowledge of crisis interventions strategies. Strong time management and organizational skills to maintain project tracking activities and manage multiple priorities. Strong analytical ability to assess various crisis situations and recommend appropriate plan of care. Ability to work independently with limited supervision. Requires a valid driver's license, acceptable driving record, minimum liability insurance coverage and the ability to be insured by the CHHS auto insurance carrier. Requires personal computer and word processing knowledge in order to enter and retrieve related information for effective reporting and correspondence.
