You are: An RN, PT, PTA, OT, LCSW or Certified Care Manager (CMC, or CCM) with significant experience in a medical setting. Passionate about care especially for older, frail, and vulnerable adults. Well established in the local eldercare community and enjoy networking, outreach, and marketing to fellow healthcare professionals like case managers, discharge planners, social workers, and administrators. Very familiar with the home care industry, have worked in home care, or have referred cases out to home care organizations. You are also a learner always looking to improve and to grow your skills and will embrace coaching and training. Emotionally strong and can work through difficult family dynamics and challenging dementia cases. Willing to meet client needs while in their homes conducting assessments, Caregiver introductions and training, and quality assurance visits.
