Responsibilities That Make an Impact: Provide comprehensive case management and needs assessments for CCBHC participants. Coordinate vital wrap-around services and linkages to internal and external resources, ensuring access to housing, mental health, medical, and social services. Support individuals in acquiring rapid benefits and presumptive eligibility for necessary medication and services. Foster community networks and assist in developing plans for overall wellness and recovery. Actively engage in team meetings, training sessions, and community-based planning meetings. Your Qualifications: Education: Associate's or Bachelor's degree in Human Services or Social Work preferred, along with a valid and clean driver's license. Experience: Two years of case management experience working with client populations; familiarity with recovery-based concepts is advantageous. Skills: Proficiency in assessments, care plans, database systems, and community resource knowledge. Strong written and oral communication skills. Professional Characteristics: Empathic, ethical, adaptable, responsible, and dedicated to continual growth and learning.
