Provides supervision to staff of care topatients in the facility. Provides services in accordance with Glen Park’s philosophies, standards, andpractices.A. QUALIFICATIONS1. Graduate of an accredited School of Nursing.2. Legally-Currently licensed by the State Board of Nursing to practice as a Licensed VocationalNurse.3. Experienced as an LVN with one of the following: a. 1 year of acute care experience.4. Current CPR and First Aid Card.5. Health/physical clearance within 6 months; TB test clearance.6. Criminal Background clearance.7. Good written and verbal communication skills.B. Duties1. Responsible for planning, organizing, directing, evaluating, and overseeing the provision ofresident care activities within the facility2. Ensuring that subordinates provide the consistent delivery of timely and appropriate high-qualityassistance, care, and medication management to all residents.3. Assist with the med pass.4. Ensuring appropriate documentation and reporting of resident condition.5. The position further serves as a facility representative on resident care matters, interfacingdirectly with outside medical professionals, regulatory agencies, and suppliers.6. Perform assessments on Admission and residents upon discharge from hospital settingsperiodically.7. Responsible for achieving specific operating, compliance, program, and client satisfaction resultswithin cost and schedule constraints.8. Assist the Administrator with various tasks relating to staff training in relation to care topics andmedication administration topics and monitoring of care staff as needed.9. Assist the Administrator with various tasks relating to training and monitoring of care staff asneeded.10. Attend daily stand-up meetings and monthly case management meetings to report care updateson all residents.11. Monitor residents and make sure they DO NOT have prohibited health conditions. See policy.12. Maintain the Weight Record of all residents & report sudden weight changes to Administration.13. Complete Weekly Progress notes for residents.14. Monthly head-to-toe assessments for all residents.15. Perform assessments on Admission and residents upon discharge from hospital settingsperiodically.C. LINES OF SUPERVISION1. SUPERVISION GIVEN & TO WHOMa. Has authority to delegate responsibilities to the Direct Care Supervisor/Med Aide whileadhering to Title 22. 2. SUPERVISION RECEIVED & TO WHOMa. Delegated by the Administrator, authority:b. To practice as an LVN with managerial and clinical in nature. D. PHYSICAL REQUIREMENTS1. Prolonged periods of standing, bending, and lifting.2. Able to lift 100 pounds to assist fallen residents.3. Able to lift 10 pounds for routine performance of essential functions.4.
