Position: Prenatal Outreach Supervisor Classification: C43 (Exempt) Office: Health Promotion Department: Prenatal Outreach Hours: Full Time M-F 8 am - 4:30 pm * Salary commensurate with experience Position Summary: Under the direction of the Maternal & Child Health Senior Manager, the Prenatal Outreach Supervisor is responsible for developing, recommending, and implementing goals and objectives for the Moms and Babies First Prenatal Outreach Program to reduce infant mortality in Montgomery County. Minimum Qualifications: Bachelor's Degree in Social Work, Public Health, Health Science, Sociology, or related field Two (2) - Four (4) years of project management and supervisory experience including home visits Knowledge in the areas of basic health literacy, home visiting care management, grant writing and performance outcomes Excellent verbal and written communication skills Understands and acknowledges that Public Health - Dayton & Montgomery County uses evidence-based and evidence-informed public health practices based on scientific research including health promotion theory and practice, environmental health measures, and communicable disease control measures including vaccination, isolation, and quarantine practices, and medicine for treatment and prevention of disease, among others. Understands and acknowledges the use of tobacco or nicotine products in any form including replacement therapy products and prescription medications used to treat smoking addiction for any anyone hired after January 1, 2014, is prohibited Required to play an active role during the public health emergency, crisis, outbreak, incident, or event in which the Health Commissioner deems additional personnel resources are warranted. This may include changes in responsibilities and working locations/hours Completion of Incident Command System 100 and National Incident Management System 700 courses within 90 days of hire Valid State of Ohio driver's license and insurance on any personal vehicle that will be used for work Preferred Qualifications: Master's degree in Social Work, Public Health, Health Science, Sociology, or related field Five (5) years of project management and supervisory experience including home visits Community Health Worker Certification Responsibilities include but not limited to: Coordinating, coaching, and supervising Community Health Worker, and Office Assistant staff by assigning program tasks to meet agency and program goals; identifying problems and resolutions by continued leadership through training, discipline, accountability, and evaluating performance; serving as a liaison between staff and leadership team Developing program logic models and work plans based on objectives and goals Reviewing and submitting program related reports to meet grant requirements Screening client referrals to determine eligibility Conducting home visits with staff as needed Overseeing financial activities for the department/program; assisting in the development of department and/or program budgets Conducting annual chart audits and monitoring program outcome measurement and evaluation Writing and managing grants related to program services Adhering to compliance protocol with Federal, state, and local regulations; and is responsible for rule, code, and policy implementation.
