Rapid Rehousing Case Manager Job DescriptionPosition Classification & CompensationThis role is classified as an exempt, full-time employee as those terms are defined in the Center’s Employee Handbook and is eligible for employee benefits as defined for this classification.Reports to: Housing and Outreach Program ManagerSummary: The Case Manager assists with the implementation and development of the Housing and Urban Development program by developing housing opportunities, partnerships, and a wide range of supports for homeless youth ages 16-24. The scope of work is subject to change as needed to ensure the project’s success and to meet funders’ requirements. Likewise, complementary changes may be made to projects or individual goals and expectations for employees based on agency/management strategic goals. All Center for Children team members are expected to be collegial and collaborative, mutually supportive, and focused on excellent services to clients, fidelity to program models, and ethical provision of services. Primary Duties and Responsibilities• Becomes expert on RRH program standards via resources on the Health and Human Services website and the Runaway and Homeless Youth Training and Technical Assistance Center (RHYTTAC). Takes the lead on compliance reviews.• Participate on the Performance and Quality Improvement processes ensuring services are being delivered with fidelity to model and contractual/licensing obligations. • Adhere to all agency standards, licensing standards, and other policies/regulations.• Become quickly familiar with HUD Rapid Rehousing rules and regulations in order to administer compliant programs.• Embrace a positive youth development and trauma-informed approach to practice.• Works with leadership team to craft and execute strategic plan for targeted outreach and partner development to meet each grant and program's goals. • Works within marketing and outreach budgets to maximize visibility in the community and access targeted populations, including homeless youth, youth at imminent risk of homelessness, families willing to serve as host homes, and landlords willing to house homeless youth and young adults. • Recruit and screen host home families and landlords in accordance with policies and procedures, ensuring low barrier housing opportunities and opportunities that are receptive to the LGBTQI population and pregnant/parenting teens. • Provide intensive case management, linking clients, based on their preferences and individual strengths/abilities, with the housing option of their choice and with educational, transportation, health and behavioral health, and basic services providers that exist in El Paso County. Case management for this population includes:o Conducting intakes, assessments, and creating care plans for identified/referred clients.o Prospecting for partners who can provide core supports including food, clothing, furniture, educational opportunities, transportation, and other assistance to homeless youth.o Connect and build relationships with other care providers in the region including McKinney-Vento liaisons and other personnel in schools, homeless service providers, the Homeless Coalition, and others who can support the success of the project and the youth served.o Consistent, frequent visits to clients to ensure health and well-being, offer support and guidance, and work with clients toward independence.o Writing and/or overseeing drafting of proposals, reports and other correspondence with prospects.o Outreach through social venues and areas of neighborhoods that attract targeted youth, including on evenings and weekends.o Engaging families in care in efforts to reunite homeless youth with kin caregivers and mediate terms of reconciliation.o Completes discharge and aftercare planning preparation and documentation. o Participate in on-call rotation to support landlords, youth, and Host Home families for non-working hour emergencies.o Adhere to reporting requirements for any suspected abuse/neglect of minors who are clients or the children of clients. o For minor youth in housing staff housing placement decisions with leadership team members.o Document and track all recruitment and case management activities. Documentation includes entry into various systems such as the Homeless Management Information System (HMIS), the Runaway and Homeless Youth Information System, and Extended Reach.• Work closely with the Project Coordinator to ensure all facets of the project are carried out with fidelity to the approved proposal and in compliance with regulations. • Conduct events and activities for participants, engaging partnering agencies as feasible.• Provide education and presentations to community stakeholders to raise awareness about the warning signs that a youth may experience homelessness. Skills• Knowledge of Housing and Urban Development (HUD) regulations. • Outstanding interpersonal skills and the ability to relate well to diverse individuals at all organizational levels.• A strong results orientation, openness to new ideas, and a proven ability to deliver a consistently high quality of service to both external and internal work partners.• Excellent written and verbal communication skills.• Problem solving skills are essential for developing creative solutions to program/personnel challenges as they arise.• Ability to be flexible and adapt to changes in staffing, program scope, assigned tasks, and modalities utilized, setting the example for team members as changes are made.QualificationsMinimum qualifications for this position include:• A bachelor’s degree from an accredited college or university in social work or other human services field; preferred.• Must have at least two years of experience serving at-risk or homeless youth/families.• Ability to read, write, and speak Spanish and English is preferred.• Exceptional computer skills, including proficiency in the use of the Microsoft Office Suite of business applications: Word, Excel, and PowerPoint.• Good time management, organizational, and interpersonal skills, and ability to communicate effectively with individuals from all walks of life. • Must have driver’s license, vehicle in safe operating condition, and maintain proof of current insurance at Texas minimum standards.• Must be able and willing to travel for trainings/meetings/conferences.• Must be able to walk, sit, stand, and lift and carry at least 25 pounds.Work Environment• Housing environments, working in communities, and conducting outreach, visiting apartment complexes. Travel across community and to areas where youth reside. The scope of work is subject to change based on the team’s performance and the funder’s expectations and on agency/management strategic goals.
