Job Title: Cumberland River RHOAR Nurse Summary of Job Duties : The Nurse will evaluate the level of medical wellness of an individual at risk of medical complications. The nurse will train direct care staff, will have administrative and supervisory responsibilities, as well as coordinate intakes with the SOS supervisor and staff. Qualifications : To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. This position requires RN Nursing degree. A minimum of one year of supervisory experience is preferred. Knowledge of women’s issues in recovery is helpful. If in recovery, a minimum of 2 years continuous abstinence from drugs/alcohol required. Submit to and pass a criminal record check and a drug screening test. Hours : Monday through Friday 8:00 am until 4:00 pm. Evening and weekend hours may be required. This position reports directly to the Cumberland RHOAR Program Director. Essential Duties and Responsibilities: Include the following but NOT limited to: Write a staff note for each service provided. Respond to emergency situations in a timely fashion. Screens all clients for admission and maintains a waiting list. Coordinate new intakes into program. Demonstrate the ability to work effectively with other team members, as part of a multidisciplinary team, and independently, when necessary. In cooperation with team, creates and maintains a consistent and nurturing environment for all clients. Develop, implement, and monitor goals and qualify outcomes for personal professional growth and development. Other duties such as providing health care screenings and health services to the clients. May be required to perform drug screenings on clients as needed. Develop forms as needed, i.e. wellness monitoring, health screening. Recommend/assist case managers/coordinators with referrals and follow-ups based upon observations of the health screening process. Review at least weekly each clients’ medical record for quality assurance and completeness as it is relevant to medical information (i.e. MAR, allergies, RX, medication logs, documentation, OTC, medical history, crisis plan, physical, health screening). Works regionally with police, medical personnel, and other community service agencies and individuals to provide and promote optimal services to clients. Provides daily supervision of operations to ensure accurate, complete, and up-to-date records are kept and all clientele are receiving appropriate care. Provide regularly scheduled trainings and documented competency for all staff administering medication: (a) Medication Administration, storage, and documentation, (b) Medication and Seizures (c) Other components could include: self-medication, family education about medication and side-effects, proper reporting, causes and effects on specific individual’s medication and unique medical situations. Administer, supervise, and document proper disposal of medications. Review medication error incident reports and follow-up. Document results or follow-up process. Will receive all MARs by the 5th of the following month, count total doses, and include this data on the monthly medication error report and the accumulative total doses form. Will perform daily medication counts to ensure all clients on medication are taking as prescribed. Collaborate with case manager to develop a plan for medication reduction when medications are used for behavior, which will be submitted for approval to the prescribing physician. Work in conjunction with physician, health dept., clinics, etc. to provide individualized staff trainings for consumers with diabetes, colostomy care, catheter, use of suppositories, etc. Collaborate with prescribing physicians to develop an individualized medication protocol for medication errors such as missed dosage, etc. instead of contacting the doctor for all medication errors. Assist the Case Manager/Coordinator in the referral process for the request for specialized medical equipment and supplies per physician’s order. Review, revise, and develop policies and procedures as they relate to wellness monitoring, medication, training, etc. Participate in the review and analysis of incident reports. Capacity to consult directly with psychiatrists to possibly prevent inpatient treatment. May administer shots per request from human resource office. Attends and participates in all required meetings. Other duties as assigned. Required Job Skills : Oral and Written Communication Skills Interpersonal Skills Ability to exercise good judgment, tact, diplomacy and compassion when problem solving, handling conflict or in a crisis Ability to complete record keeping in a timely fashion Ability to formulate treatment objectives for the person served Ability to get along with diverse personalities and maintain effective working relationships Ability to work well with people of all races, backgrounds and needs Knowledgeable of Behavioral Health Concepts and how to incorporate concepts into practical application Ability to work as part of a comprehensive community mental health team and to positively represent the agency in the community Knowledgeable of the stages in recovery for behavioral health and/or chemical dependency A current Kentucky License as a Registered Nurse (RN) Supervisory/managerial experience (preferred) Clear Background Checks Valid Social Security number Negative drug test Additional Responsibilities: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Possess a valid Driver’s License, maintain a safe driving record, and maintain the required minimum vehicle insurance amount for the State of Kentucky, if required for job performance. Environmental Data and Job Hazards : Hours of work are structured 8.0 hour blocks of time but may be subject to irregular and/or flexible hours based on needs or demands. Position may require duties to include routine or reasonable anticipated tasks or procedures where there is a degree of actual or potential exposure to blood or other infectious materials. All staff are required to participate in yearly trainings regarding infection control and safety in the environment including hazardous waste and blood borne pathogens. All new staff must attend training regarding handling physical, verbal threats, acts of violence or other escalating and potential dangerous situations including when police need to be summoned. All staff must be certified in Cardiopulmonary Resuscitation within one month of employment and maintain the certification, if providing direct care. Powered by JazzHR
