Registered Nurse physician employment in Massachusetts : Home Health RN Case Manager ( Multiple Locations ) LOCATIONS Merrimac, MA Dracut, MA Arlington, MA Wilmington, MA Salary Range: $87,000 - $93,000 Per Year Our fantastic home health company is growing and looking for nurses to grow with us. Must know Homecare Homebase, Point Click Care, and OASIS Health Home healthcare nurses provide one-on-one care for patients in their homes and may involve wound care, medication management, medication administration, hands-on care to patients, educating, observing and monitoring patients' conditions, maintaining accurate records and making physician recommendations.. This is a self scheduling opportunity with full-time benefits in which you may be providing: Assess all medicines to identify possible ineffective drug therapy or adverse reactions, significant side effects, drug allergies, and contraindicated medications. Establish realistic measurable, observable goals consistent with the patient's diagnosis. Comply with applicable legal requirements, standards, policies and procedures including, but not limited to Compliance Process: Code of Conduct, HIPAA, and documentation standards. Demonstrate professional conduct, service excellence Qualifications REQUIRES NH LIC (company reimbursement) Active (unrestricted) nursing license in the State of MA Active Driver's License 1 year (professional) nursing experience in Home Health episodic visits Home Health Case Management experience a plus (hospital CM is not the same) Currently working in HH and know, HCHB, OASIS & PCC
