Location: Larimer County, CO Status: Full-time, 40/hrs. week What You'll Do: As part of SummitStone's commitment to integrated care, this position entails working in partnership with the UCHealth-led Medicaid Accountable Care Collaborative (MACC) program and team. The MACC program provides community-based care coordination and supportive services to clients with complex care needs. This case manager position provides outreach for new referrals, ongoing assessment, crisis intervention, moderately intensive care coordination / case management, community-based care planning, transitional care support, and support with linkage to relevant social and community resources / services. MACC is a multi-disciplinary team working with clients who have complex care needs, including chronic medical conditions, addiction / behavioral health challenges, and/or significant barriers to accessing care and services. The Behavioral Health Consultant (BHC) /case manager will manage a caseload of MACC clients, including conducting outreach to newly referred clients, completing intake assessments, developing and monitoring care plans, monitor treatment progress, and document client encounters / maintain clinical records in an electronic medical record (Epic).Primary responsibilities of the BHC / case manager include coordination of care with internal and external providers, agencies, and community partners to support the health-, behavioral health, and social needs of clients. Overarching goals of the MACC program include improving health outcomes, decreasing barriers to accessing care or services, and avoidance of potentially duplicative services or preventable health-related events. Essential Duties: Provide care coordination and supportive services to clients as assigned by the Program managers. Provide/facilitate a variety of individual, family and group therapeutic and rehabilitative activities to achieve goals of care plan; prevent deterioration, and / or hospitalization. Conduct crisis intervention via telephone or in-person / face-to-face. Intervene and de-escalate crisis situations with clients. Assess risk of harm to self or others and arrange needed resources. Observe clients in a variety of situations; assess mental status; formulate care plans and goals; perform care coordination/case management services as needed to assist client to meet their goals as stated in their care/treatment plans; assist clients/consumers in accessing supportive services such as vocational, medical, social security, housing authority, etc. Advocate for consumers in many different forums including within the medical community, at the federal, state and local court systems, with family, school system, etc. Assist consumers in improving their health, well-being and quality of life by training in behavioral skills; educate consumers to make informed choices; develop clients coping skills; help consumers to problem solve and negotiate tasks of daily living and managing health care needs. Document care/treatment plans, clinical progress notes, care coordination and case management notes, letters, faxes and phone calls. Follow proper protocol for documentation as set forth by SummitStone Quality Assurance, program stakeholders, and to third party payors' requirements that have contracts with MACC. Maintain close and cooperative relationships with community agencies and other public agencies in carrying out care/treatment plans. Follow agency productivity standards as outlined by SummitStone Health Partners, and UCHealth. Perform other duties as assigned. Required: Relevant Bachelor's degree (i.e. in social work, human services, or related field) Preferred: Two years' clinical or care management experience Direct-practice client work in an integrated-care clinical setting such as a clinic or hospital Master's degree in Social Work, or other human services / health-care related field Bi-lingual / bi-cultural Training in addictions work and/or trauma-informed care Experience working with medical and/or Behavioral Health providers Some positions include driving a company vehicle and require a valid Colorado driver's license.
