Full Primary & Family Care, Peds, BH, Case Management, Dental, Women's Health including Pre & Post-Natal, Peds, On-site Lab, Chronic Disease Care/Mgmt, Preventative, Lab, Health Education, Immunizations, Transportation, Enrollment, Referrals and Specialty Services are all available to patients and a great one stop shop for patients.Opportunity Highlights: Environment : Mission-driven, patient-centric culture, team-based, out-patient Services You will provide: D elivering empathic and professional care to children and adolescents (birth to 18 years). Responsibilities include routine preventative care, acute and chronic condition management, and age-appropriate medical education.
