This position is eligible for American Rescue Plan Act (ARPA) funds which includes a one-time $750 sign-on bonus payable after the completion of 90 days of employment as well as a monthly stipend of up to $694.50 which is based upon the hours worked in the previous month for as long as the funds are guaranteed by the State. About Your Team Your team understands the impact of trauma and provides residents and their families with solutions for positive outcomes and permanence. Your team creates a sense of stability by providing a home-like and nurturing environment with daily experiences and life skills. What You’ll Be Doing You will provide appropriate role modeling in a trauma-informed therapeutic environment that promotes the physical, mental, and behavioral health of each resident and assists them in developing life skills. The Residential Services Case Manager works as a part of a collaborative trauma informed therapeutic team, establishing open communications and support among co-workers, and provides case management services support and advocacy for residents. Your Main Priorities While additional responsibilities are likely to arise on the job, you will primarily:• Provides care coordination for all residents in the program related treatment team meetings, mental health, health, dental, and all necessary appointments. • Participates in resident interviews, intakes, and schedules treatment team meetings for each individual. • Communicates with stakeholders regarding resident updates and coordination of care. • Documents all communication with providers in appropriate systems pertaining to status of resident and interactions. • Understands short and long-term impacts of trauma and neglect to cultivate a trauma-informed environment. • Collaborates as a member of the team of therapeutic staff in promotion of residential services philosophy and program protocols. • Ensures active engagement in day-to-day structured and unstructured recreational activities, psychoeducational groups, regular resident house meetings, and life skills development and maintenance. • Implements crisis intervention techniques in accordance with policies and procedures, and agency training; ensures appropriate interventions are used if needed. • Maintains positive and professional interactions with program and community partners (i.e., parents, police departments, DCYF, child advocates, etc.). • Responsible for transporting residents by agency vehicle in accordance with state and agency transportation policies. Who You Are Requirements, Skills & Abilities:• Associate degree in social work, Human Services, or related field required; bachelor’s degree in social work, Human Services, or related field preferred.• Minimum 6 months’ experience working with children and/or adolescents required; experience working in a group home setting preferred.• Bilingual in Spanish preferred. • Strong organizational, critical thinking and time-management skills.• Basic computer skills required; knowledge of Microsoft Office 365.• Ability to communicate and effectively interact with a diverse population.• Collaboration and teamwork. • Must have a valid driver’s license, proof of auto insurance and car registration.• Must authorize Child & Family to complete a motor vehicle records check. • The ideal candidate will be required to meet all background checks and medical clearances required for the position. Physical Requirements Direct Service Staff (Travel as part of position):This position requires time sitting, standing, walking, driving, carrying and lifting up to 25lbs. This role also requires operating desktop, laptop and/or cell phone, and communicating with colleagues. Travel Requirements Statewide travel required.
