Therapist for FAM Program (Families are Made) Hartford, CT AreaFee-for-Service NON-CRISIS OUTPATIENT LEVEL OF CARE/ CLINICIANS ARE NOT ON CALL THIS IS NOT YOUR TYPICAL IN-HOME PROGRAM Part-time - Hybrid (In-Home & Telehealth) Company Stokes Counseling Services is a leader of mental health private practice, and group practice within the state of Connecticut. We are a group practice made up of licensed clinicians ( LPC, LMFT, LCSW, LPCA, LMSW, MFTA ) who provide an array of services including individual, family, and group counseling. FAM is an in-home program, we also offer virtual services through telehealth for those areas in which we don’t offer in-home services, and in many cases, we offer a hybrid model. Unlike other in-home programs, FAM is an outpatient level of care. With the FAM program, our clinicians are matched with a family and have the ability to do individual treatment with as many treatments as needed in addition to the family piece. We are very flexible with what treatment could look like, the clinician will do an assessment and together with the family will structure treatment in a way that will be most beneficial to the needs of the family. This is an opportunity to work a Hybrid Model where you work Telehealth AND do in-home work. Responsibilities Develop service or treatment plans based on assessment of strengths, needs, collaboration with referral agencies, and collaboration with the individual served. Provide activity and participation-based interventions and approaches developed from training materials, experience, and individual and/or families' successes. Make referrals to services and resources on behalf of individuals and/or families served. Assure that documentation and records are kept confidential, organized, and submitted to the supervisor in an accurate and timely manner. Participate in team meetings with peers and collateral team meetings when needed. Support peer development by addressing areas of concern for professional development and team performance issues. Provide an aftercare plan for each family/individual served. Build and maintain positive relationships with community partners from referring/ collateral agencies. Guide the family in their interaction with referring agencies, and other community organizations as needed.
