We take pride in working together as a compassionate team in our family orientated environment; and are looking to expand our team with a Case manager (RN) to allow for the highest level of care for our residents. As our Case Manager you will have an opportunity to use the full extent of your experience, knowledge and skills to inspire our patients to progress each day. The successful candidate will be accountable for the full patient care cycle. Candidate should be a strong, competent, compassionate leader who has the drive to achieve high clinical standards.
