A DAY IN THE LIFE OF AN RN CASE MANAGER As an RN Case Manager, you make sure that our patients receive the highest quality nursing care. To get our patients the care that they need in a timely manner, you complete initial assessments that identify the need for hospice care within 48 hours of their election, then work with the attending physician and care team to complete a comprehensive assessment. Once assessments are complete, you coordinate and initiate the plan of care. You maintain ongoing assessments with patients and revise their plan of care based on new data and changes in their condition, including physical, psychological, social, and spiritual factors that may influence their health. To maintain continuity of patient care, you communicate regularly with the attending physician and other members of the interdisciplinary care team. You also keep up-to-date patient records and complete required documentation to meet regulatory requirements. You provide essential leadership for the care team by instructing caregivers and volunteers as well as supervising LPNs and hospice aides. Always keeping the safety and wellbeing of our patients your top priority, you practice infection control precautions and report any form of abuse that you recognize. You also provide direct care as needed and assist with discharge procedures. QUALIFICATIONS FOR AN RN CASE MANAGER Oklahoma Registered Nurse license 1 year of clinical experience Reliable transportation and vehicle liability insurance Hospice, home health, critical care, or emergency room experience is preferred but not required. Can you adapt to changing circumstances? Do you enjoy coaching and mentoring others? Do you have respect for and patience with ill, disabled, elderly, and emotionally upset people? Do you have excellent interpersonal and communication skills? Are you creative and able to think critically? Do you conduct yourself with integrity and take initiative?
